Having issues with requests and tasks containing blank fields

I’m seeing two behaviors that happen randomly:

  • 1) When submitting a service request (SR), some SRs get submitted with the “Classified by request class” field remaining blank. This happens for 1 in 90-100 SRs submitted. There’s no pattern to it.
  • 2) Some work tasks have come in with no primary location. Upon checking, the SR had the floor and space cleared so only the building was left. The building then misses the association string or “Location requested for” and so does not carry over to the work task. This also does not happen consistently and is not replicable unless it actually happens with a user every once in a while.

One observation is that right at the time of submission of the SR, the following was written in the logs. Could this be a clue?
